Many of us have made sure that our children, spouses, or parents have taken medications as prescribed by a doctor. That is what assisting a resident to take medication means: providing assistance to the person to take the medication safely. Because you may be assisting more than one person and each person may be taking multiple medications, providing assistance safely can be complicated. 1. RIGHT RESIDENT. Epidural analgesia is a therapeutic modality that offers an alternative to traditional methods of pain control. It can be administered as an intermittent injection or a continuous infusion of opiate into the epidural space. The nursing management of clients receiving epidural analgesia to manage postoperative pain is an essential for client comfort and safety. In general, the client should understand the purpose and the benefits of epidural analgesia and provide consent to the modality before surgery. With the client in a side-lying position, the epidural catheter is inserted, most commonly at the L2 level, by the anesthesiologist. Once the catheter is tested for placement, it is stabilized with an occlusive dressing. It should be taped in such a manner that the tubing does not pull or kink. The anesthesiologist should be notified if the catheter becomes dislodged or the dressing becomes loose or wet. The catheter may be connected to a continuous infusion or capped and should be carefully labeled "for epidural use only." Because some preservatives might be toxic to neural tissue, medications for epidural use must not contain preservatives. Although rare, respiratory depression can occur with the administration of spinal opiates. The respiratory status of the client receiving intermittent epidural analgesia should be monitored once each hour for the first 12 hours or until the respiratory rate exceeds 12 breaths min, then every 2 hours for 12 hours, and then every 4 hours for the duration of therapy. With continuous infusion of the analgesic agent, the client should be monitored every hour for the first 24 hours or until the respiratory rate is greater than 12 breaths min, then once every 4 hours for the duration of therapy. Auscultate respiratory status with stethoscope. Clients with low respiratory rates should be monitored as necessary. Monitor pulse oximetry and arterial blood gases ABGs ; as ordered. Naloxone is usually kept in the immediate client environment according to agency policy. The anesthesiologist is called immediately if respiratory depression occurs. Naloxone 0.4 mg IV is usually ordered, but this dose may also reverse the analgesic effect of the opiate, and the client will experience pain. Have Ambu bag, oxygen, and suction in working order nearby. Maintain IV access for emergency drugs. Pruritus is experienced by approximately one-third of all clients receiving spinal opioids. Although the mechanism is unclear, this reaction is believed to occur when epidural opiates produce sensory sensations that are interpreted by the sensory cortex as itching. Low doses of naloxone are administered to relieve the pruritus without concomitant reversal of analgesia. Nausea and vomiting may also be an issue with some clients. Fluid balance monitoring and the assessment of bladder distention should be maintained while the client is receiving epidural analgesia to detect any urinary retention that may occur. Monitor dressing and site every shift. Using a pain scale, the client's level of comfort should be assessed every 2 to 4 hours for the first 24 hours after surgery. Supplemental parenteral analgesics may be required for "breakthrough" pain. Assess the level of consciousness every 2 hours for the first 24 hours and then every 4 hours for the duration of therapy. Some agencies require a motor and sensory assessment at least every 2 hours and before ambulation. Blood pressure and pulse should be determined before ambulating the client for the first time. Instruct the client to notify the nurse if pain is experienced and if increased drowsiness and or leg numbness and weakness occur. Epidural analgesia is not usually continued past the fifth postoperative day!

Owners operators of tanning salons must be aware of and adhere to the pertinent regulations under the federal Radiation Emitting Devices Act RED Act ; . In addition, operators should follow the guidelines listed below, which have been developed specifically for tanning salon operations. 1. It is recommended that tanning clients be informed of these guidelines and advised to consider discussing the risks of artificial tanning with their family physicians. All sunlamps sold in Canada, including tanning beds, must comply with the regulations specified for sunlamps under the federal government's RED Act. Owners must check with their equipment supplier to ensure that sunlamps, tanning beds and any associated apparatus being purchased and used in their salon, are in compliance with the RED Act. Knowledgeable operators or staff members who can inform and assist the public in the safe use of tanning devices should always be on premises during business hours. Staff should be familiar with these guidelines, and have completed with success the questionnaire at the back of this booklet. It is recommended that suntan salon operators ascertain a client's ability to tan, history of sunburns, history of skin infections, rashes or other conditions and use of certain medications or cosmetics to prevent photoxic and photoallergic reactions. Client records should be kept. This information is to be used for exposure planning and to help clients understand how these factors interact with ultraviolet radiation. People with sensitive skin who always burn and never tan should be advised by salon operator not to use tanning units and proscar and ovral, for example, lo ovarl 28 tablets. Becker, G. S., M. By importing generic drugs from Brazil, TAC and MSF have infringed GlaxoSmithKline and Boehringer Ingelheim patents. The South African constitution protects the rights to life and dignity, and by importing these medicines, TAC and MSF believe they are upholding these rights. It is worth remembering that patent infringement is a civil matter, not a criminal one. Because there are patents on these medicines in South Africa, there is no competition between companies to make and market these drugs in South Africa. They are therefore much too expensive. By importing these drugs from Brazil in breach of the patent law at much lower prices, TAC and MSF are challenging both the South African government and the pharmaceutical industry to take steps to allow generic drugs to be made available to those who need them Some pharmaceutical companies have made limited offers of supplying free or cut-price drugs to certain countries under particular conditions. However, there is no doubt they could do a great deal more. They have to stop making excuses about the lack of clean water and health facilities, and tackle the issue of allowing other companies to manufacture drugs in competition to their own products so that competition will drive down prices in poor countries. The way forward is for them to set their lawyers the task of finding a way of doing this that protects the patents they hold in rich countries. At the same time, governments must act. Prime minister Tony Blair has said he wants to lead a project to assist Africa on a scale not seen before; and chancellor Gordon Brown has already made more money available to tackle Aids in poor countries than did his predecessors here or than have his counterparts in other countries. But still far too little is being done. Even the prices of generic drugs are too high for the governments of most poor nations. Taxpayers in European nations, and North America will have to foot the bill.
