Study Design: Telephone interviews were conducted with a nationwide cross-section of State Medicaid Agency administrative personnel on: 1 ; activities for which they use information produced by eight targeted measures of quality of care, plus 2 ; their attitudes regarding the usefulness of that information, their level of understanding of the measurement methods, and other characteristics of their jobs and the policy environments in which they work. Associations between use of the information from each of the eight targeted measures and the knowledge, perception, and environment variables were evaluated with point biserial correlations and chisquare tests. In addition, descriptions were elicited of actions or situations that had been especially problematic or helpful. Population Studied: Interviews were completed with 82 subjects who were State Medicaid Directors, Directors of Medicaid Managed Care programs, or Managers of Quality Assessment Improvement units from 48 State Medicaid Agencies. Principal Findings: A large majority of subjects reported positive attitudes in general towards assessing quality of care, familiarity with a wide variety of assessment methods, and using information about quality of care in several different administrative activities. However, most reported using information from only a small number of types of measures, primarily enrollee satisfaction surveys and utilization encounter data. The two factors most strongly associated with use of information from a particular measure, or not using it, were 1 ; subjects' ratings of the overall usefulness of the measure for assessing quality of care and 2 ; the purposes for which subjects used information about quality of care. In addition, subjects described several common problems they have experienced in attempting to use specific measures. Some problems were inherent to attempting to use certain methods to measure quality of care, though many were related to implementation difficulties, such as inadequate technical expertise. Subjects who described actions that had been helpful in overcoming those problems frequently mentioned the importance of using several different types of measures in conjunction in order to get a more complete picture. Conclusions: These findings suggest that the primary barrier for state Medicaid administrators' use of information about quality of care in inadequacy of information that is appropriate to their needs. Also, relying upon any one measure provides an incomplete picture of the quality of care available in the program. Implications for Policy, Delivery or Practice: Selection of quality of care assessment measures that have proven feasible to implement within the structure and resource limitations of state Medicaid programs.
